PROCEDURE FOR USE OF FACILITIES (INDOOR AND OUTDOOR)
PRIORITY OF USERS
The Town of Bloomfield Leisure Services Department will provide reservations and permits for exclusive use of indoor and outdoor recreation facilities on a first come/first serve basis with priority given to:
1. Town and School Uses
2. Approved Organization Recreation Uses
3. Approved Business/Industry, Non-Profit/Institutional Recreation Uses
4. Meetings of Non Profit Local Groups

DEADLINE AND REVIEW SCHEDULE: The Parks and Recreation Committee generally meets on the first Monday of the month and therefore the request for approved status including informational material must be received at the Leisure Services Office on the Wednesday prior to the meeting for inclusion on the Parks and Recreation Committee Agenda.

GRANTING APPROVED ORGANIZATIONS STATUS.

Town of Bloomfield
PURPOSE: It shall be the policy of the Town of Bloomfield Parks and Recreation Committee to encourage and assist the formation, continuance, and growth of recreational programs and activities by Bloomfield residents, groups and organizations.
The Parks and Recreation Committee will provide encouragement and assistance by granting approved organization status. This involves a statement by the Parks and Recreation Committee
that the organization in question, along with its recreational services and activities, have been
evaluated and, subsequently, judged to be in the best interests of the residents of Bloomfield.
BENEFITS: Once approved by the Parks and Recreation Committee the organization may be eligible to receive direct non-financial assistance. Such assistance may be in the form of permits and reservations for use of facilities. (Indoor or Outdoor)
CONDITIONS: The Parks and Recreation Committee may extend approval to an organization that meets the following conditions:
A. The organization must be a non-profit Bloomfield organization.
B. The group or organization, as well as the activities and programs conducted,
must be open to all residents of Bloomfield on a fair and equal basis. There
must be a democratic process and provisions for electing officers.
C. The group or organization must give evidence of its organizational structure
for the purpose of establishing its ability to responsibly manage the program
or activity. A written report to the Parks and Recreation Committee should
include the following:
1. A brief history of the organization.
2. A statement of objectives and goals.
3. Constitution and By-Laws, or code or operations, or statement.
4. List of current officers and membership roster including addresses.
5. Listing of activities and programs.
6. Organizational membership requirement. The Parks and Recreation
Committee requires that at least, but not less than, fifty-one percent
of the (51%) of the members must be residents of the Town of
Bloomfield. Verification of residency is required.
7. Reason(s) for requesting approved status.
INSURANCE PROVISIONS AND INDEMNIFICATION AGREEMENT:
The Organization is required to provide the Town with:
1. A CERTIFICATE OF INSURANCE with Broad Form General Liability
Coverage naming the Town of Bloomfield as an additional insured written
on a “per occurrence” basis with limits of no less than $1,000,000 (combined single limit).
2. An LESSEE’S HOLD HARMLESS/INDEMNIFICATION AGREEMENT:

GRANTING APPROVED LOCAL BUSINESS/INDUSTRY AND
NON-PROFIT INSTITUTION STATUS.
PURPOSE: It shall be the policy of the Town of Bloomfield Parks and Recreation Committee to encourage local business firms and non-profit institutions to provide wellness, fitness and recreation programs for their employees and members.
BENEFITS: The Parks and Recreation Committee will provide encouragement and assistance by granting approved business, industry or non-profit institution status.
CONDITIONS: The Parks and Recreation Committee may extend approval to a business, industry or non-profit institution that meets the following conditions:
A. The business, industry or non-profit institution must adhere to Town of Bloomfield ordinances, and to the policies and rules and regulations of the Parks and
Recreation Committee.
B. The business, industry or non-profit institution must be located in the Town of
Bloomfield. If a business, industry or non-profit institution has a branch or
division in the Town of Bloomfield, that branch or division may qualify for
approved status for the employees or members of that branch or division.
C. If Bloomfield business, industry or non-profit institution is requesting approved
status for use of facilities, the following must be submitted in writing:
1. Documentation showing that the business, industry or institution is
located in the Town of Bloomfield.
2. The reason for the request.
3. Information on the primary users (employee or member group or club,
company or institution team, program, etc.)
4. Acceptance of sponsorship of the activity.
5. The name, address and telephone number of the contact person or person
responsible for the activity to be conducted at Town facilities.
6. Information to verify company or institution sponsorship of the activities
and the need for use of public facilities.
7. A CERTIFICATE OF INSURANCE with Broad Form Commercial
General Liability Coverage naming the Town of Bloomfield as additional
insured written on a “per occurrence” basis with limits of no less than
$1,000,000, (Combined single limit) or in such other amounts as the Town
shall require.
8. A LESSEE’S HOLD HARMLESS/INDEMNIFICATION
AGREEMENT in the form required by the Town.

LESSEE’S INDEMNIFICATION AGREEMENT
The undersigned, hereinafter referred to as the Lessee, in leasing or requesting the use of the __________________________________________(name of building or facility) agrees that he or it will indemnify and save harmless the Town of Bloomfield and its officers, agents, employees, named as co-defendant in any claim or suit, on account of any and all claims, damages, losses, workers’ compensation payments, judgments, litigation expenses and legal counsel fees arising out of injuries to persons (including death) sustained by or alleged to have been sustained by the officers, employees or agents of The Town of Bloomfield or by any officers, employees, agents of the Lessee or by any participant or spectator, or arising out of damage to property real or personal alleged to have been caused in whole or in part by acts or omissions of the
Lessee or any participant or spectator or anyone directly or indirectly employed or working for the Lessee, including volunteers, in connection with the lease or use of ________________________________________________(name of building or facility). The Lessee further undertakes to reimburse the Town of Bloomfield for any damage to its real or personal property occurring in connection with the lease or use of _______________________________ (name of building or facility) by Lessee unless the damage is caused by the Town of Bloomfield.

Leisure Services and programs are available and maintained for the use of residents of the Town of Bloomfield. The Leisure Services Department uses along with proof of identity, the Board of Education’s proof of residency criteria, one of which should be presented for verification of residence.

put a sticker on. (Minimum of 2 pieces of mail needed)

Enrollment in Bloomfield Public School or Eligibility to attend a Bloomfield Public School.
